These forms are for students who have been accepted for admission and have enrolled in the Cadet Corps. For forms needed to apply for admission, see the On-line Application page.
Enrollment Forms should be mailed to the Admissions Office before the start of school, or completed using Adobe Reader® and e-mailed to admissions@militaryschool.com. Cadets new to the Academy must complete all forms. Returning cadets complete forms to indicate information revisions. (Typically, returning cadets must update Forms 1-3 regarding family contact information and accounts, and Forms 7-15 regarding health and medical information.)
Each form must be signed by a parent or guardian before the start of school. (Forms previously emailed may be signed by parents at registration.)
Parents are encouraged to submit forms ahead of time to expedite the registration process when the cadet reports for the start of school.
On the Cover Sheet, please initial next to the name of each form, indicating that you have accurately completed and signed each form. Medical Forms 12 and 15 must be signed by a licensed physician.
