Enrollment Forms
Please complete and return the following forms to the Admissions Office before arriving on campus for Registration.
All forms are interactive and can be filled-in/typed
in using Adobe Reader and then e-mailed to
admissions@militaryschool.com. Cadets new to the Academy must complete all forms. Returning cadets need to complete those forms where any information has changed. The most common forms needing yearly update are Forms 1-3 regarding contact information and cadet accounts, also Forms 7-15 regarding health and medical information.

The forms on this page are for completing after you have
completed the Application for Admission and associated forms and
after you have been notified that your son or daughter has been
accepted for enrollment. If you are looking for an Application for

MASSANUTTEN MILITARY ACADEMY PARENT/GUARDIAN INFORMATION AND AUTHORIZATION FORMS
These forms may be
downloaded and filled in using Adobe Reader and e-mailed to
admissions@militaryschool.com. Each form must be completed and signed by the Applicant’s parent or guardian prior to Applicant being enrolled at Massanutten. The forms may not be modified in any way.
Please call us if you are unable to download the forms.
Having
the forms filled out ahead of registration day will be an advantage
for you and will speed up the registration process tremendously.
Forms
On the Cover Sheet, please initial next to the
name of each form, indicating that you have accurately completed and signed each form.
Forms 12 (if medicine is prescribed) and 15 must also be signed by
a physician licensed to practice medicine.
