Use these forms for the regular school year or for summer school.

Please fill-in the adobe document and e-mail it to admissions@militaryschool.com or print the application, sign it, and fax it to 540-459-5421 or mail it to Admissions, Massanutten Military Academy, 614 S. Main St., Woodstock, VA 22664. Please include any supporting documents.

The nonrefundable application fee is $50.

	When you have completed the application form and e-mailed/faxed/mailed it to the school, click here to pay for your application fee online.

Confidential Reference Forms (22KB each). Sign these forms and give them to your child's current school. The school will complete them and return them directly to Massanutten Military Academy.
